The following policy sets forth how Ottawa Derm Centre Inc. (“Ottawa Derm Centre”) collects, uses and discloses the information about Ottawa Derm Centre’s website users (“Users”) that Ottawa Derm Centre collects on this website (“User Information”). Information Collected

During the registration process, Ottawa Derm Centre collects identifiable information, including, but not limited to, legal names, telephone numbers, postal and email addresses relating to the User, and any other identifiable information that Ottawa Derm Centre may reasonably require in order to provide products and services to the Users (“User Information”). Users who use the ODC HEALTHCOIN REWARDS Registration Service may be assigned user names and passwords, all of which are used to identify the Users when they are using the ODC HEALTHCOIN REWARDS Registration Service.

Use of Collected Information

Ottawa Derm Centre also uses User Information to monitor compliance with the rules and policies.

Ottawa Derm Centre may compile and analyze its User Information into aggregated research reports. Except as may be otherwise agreed in a separate agreement between Ottawa Derm Centre and the User, Ottawa Derm Centre may share this aggregated data with its business partners.

User access, update or deletion of information collected by the Ottawa Derm Centre

Upon written request, Ottawa Derm Centre will provide to User a summary of any User Information retained by Ottawa Derm Centre regarding the User. Users have a responsibility to keep the certain information current. Users may correct, change, or update the User Information and information by notifying Ottawa Derm Centre in writing of such change. Ottawa Derm Centre undertakes to use its best efforts to correct the User Information within a reasonable time after receipt of such request.

Information Sharing

Ottawa Derm Centre and its authorized representatives have access to the User Information, including the Email List. Ottawa Derm Centre shares User Information with third parties when Ottawa Derm Centre has permission from the User, when Ottawa Derm Centre believes in good faith that the law requires it, or under the circumstances described below.

Ottawa Derm Centre may disclose User Information when Ottawa Derm Centre has reason to believe that disclosing User Information is necessary to identify, contact or bring legal action against someone who may be violating Ottawa Derm Centre’s Terms of Use or may be causing injury to or interference (either intentionally or unintentionally) with Ottawa Derm Centre rights or property, other Users, or anyone else who could be harmed by such activities.

The User will be provided with a choice of whether to accept or not accept this offer.  If the User accepts, Ottawa Derm Centre (or agents working on behalf of Ottawa Derm Centre) may occasionally send the User such information.  If this offer is not accepted, then the User will not receive such information. 

Withdrawal of consent to collect, use and disclose your personal information may restrict our ability to provide you with some products and services, such as the privileges and opportunities of being a User of ODC HEALTHCOIN REWARDS Loyalty Program https://www.heathcoinrewards.ottawadermcentre.com. You may always withdraw or refuse your consent by clicking on the unsubscribe link in our email communications or by email at info@ottawadermcentre.com. Your request will be promptly processed, but it may not be in time to remove you from promotions already in progress. Please note that even if you have opted out of receiving marketing communications, we may still contact you for the purposes of administering your account and sending you transactional or operational emails.

User Contact

Ottawa Derm Centre reserves the right but is not obligated to contact Users regarding account status and changes to the ODC HEALTHCOIN REWARDS Registration Service, this Privacy Policy, or any other policies or agreements relevant to the User.
